Among the documents you will need is a copy of your child's birth certificate.
ANY PERSON BORN IN MISSOURI after 1920 can receive a certified copy of their birth certificate at the office of the:

The fee for the certificate is $15.00. The certificate will be given to you immediately.
A COPY MAY BE OBTAINED THE SAME DAY BY APPLYING IN PERSON BETWEEN THE HOURS OF 8:30 A.M. AND 4:15 P.M. MONDAY THRU FRIDAY.
PHOTO IDENTIFICATION IS REQUIRED
For out of state Birth Certificates, contact St. Charles County Health Department for forms.
